Supports Planner/Case Manager
Garrett County Community Action Committee, Inc. / Aging and Nutrition is seeking a Supports Planner/Case Manager for In-Home Care services. The right person for this position will be instrumental in helping clients develop a bundle of services that will support them in their home. They will monitor services provided, report on services provided and track time for billing purposes. The position requires a Bachelor’s degree in Gerontology, Social Work, Sociology, or related field. Comfortability with technical skills in data management and reporting, computers and writing a plus. This is a fulltime position 40 hours /week with full agency benefits.
