Supports Planner/Case Manager
 
Garrett County Community Action Committee, Inc. / Aging and Nutrition is seeking a Supports Planner/Case Manager for In-Home Care services. The right person for this position will be instrumental in helping clients develop a bundle of services that will support them in their home. They will monitor services provided, report on services provided and track time for billing purposes.
 
The position requires a Bachelor’s degree in Gerontology, Social Work, Sociology, or related field. Comfortability with technical skills in data management and reporting, computers and writing a plus. This is a fulltime position 40 hours /week with full agency benefits.

Flowery Vale Health and Fitness Center Aide
 
Garrett County CAC/Aging and Nutrition is seeking a dynamic person for the position of fitness center aide or Flowery Vale Health & Fitness Center for Mature Adults in Accident, MD.
 
This is an ideal part time position for someone who values lifelong health and fitness. Duties include monitoring health/fitness classes and fitness equipment activities and maintaining the cleanliness of facility. Knowledge of senior fitness activities, experience with older adults and basic computer skills are a plus. This is a permanent part time position 10.5 hrs/week at $12.71/hr. with potential for an additional 6 hrs/wk.

Coordinator II
 
Garrett County Community Action Committee, Inc. Service Coordination Department is accepting applications for the position of Coordinator II. (See Cooordinator II for official position description).
 
Responsibilities for the successful applicant includes but not limited to coordinating customer access to available programs and resources by accessing family needs, coordinate client access to assigned programs, assist individuals gain information and access to health exchange information, provide case management coordination, serve on family centered teams, are familiar with the range of service programs available within the county. Must have a valid driver’s license and access to a vehicle for outreach.
 
Education requirements are AA Degree preferred, high school diploma or GED required. Basic computer knowledge required. The position requires knowledge of local resources. Must have the ability to create realistic plans with families and build rapport with wide range and diversity of people. Position is full time with benefits. Criminal background check is required.
